Sponsored TV form submission deadlines

• October 24th (fulfilled by 10/31)
• November 18th (fulfilled by 11/25)
• December 4th (fulfilled by 11/12)
• December 15th (fulfilled by 12/22)

Sponsorship submitted request fulfillment
